Liability and Claims *

I waive for myself, and my heirs, executors, and administrators, any and all rights and claims for damages, demands and any other actions whatsoever, including but not limited to personal injury and death, which I may have against the United States and/or its officers, agents. employees, and instrumentality’s which may occur as a result of my participation in the aforementioned Ramstein Outdoor Recreation activity.

Physical Fitness and Skills Acknowledgment *

I certify that I am physically fit for participation in the above activity and that I will complete a skills briefing and I also agree that I will not participate in the outing if I do not understand what is briefed or I am not physically capable of safely accomplishing this activity. I understand that participation in this activity may subject me to potentially hazardous or physically strenuous activities and I understand the need for extra caution on my part.

Indemnification and Hold Harmless Agreement *

I further agree to indemnify and hold harmless the United States and its officers, agents, employees, and instrumentality's including the Outdoor Recreation Program of the 86th Force Support Squadron at Ramstein Air Force Base, for any injury or damage to United States personnel or property, and for any suits and damages filed against the United States which may result from my participation.

Responsibility for Additional Expenses *

I also agree that any additional expenses incurred by the Outdoor Recreation staff on my behalf, such as additional lodging or meals due to weather delays or other emergency situation will be provided on a reimbursable basis and I may decline such services to avoid being charged for them.

Photo Release

I agree that I can have video and photos taken of me/my child by 86 FSS personal for official use. I understand that 86 FSS will only use photos or videos for official marketing and will not depict me/my child in a negative light.
